Health fund reclaiming (HICAPS):
Bring your health fund card with you to your appointment for easy claiming through our HICAPS terminal.
The amount reimbursed from your fund depends upon your level of cover.
Please contact your fund prior to your appointment if you require details regarding this amount.
You may then pay the remaining gap by EFTPOS, credit card or cash payment.
GP Chronic Disease Management Plans:
Your GP has provided you with a Chronic Disease Management Plan that may allow you to get a Medicare rebate for up to 5 physiotherapy sessions.
Please bring this form with you on your initial visit.
As we do not bulk bill please note that there will still be a gap payment required as the Medicare rebate will reimburse you back only a portion of the session cost (approximately $61.80).
You will be required to pay the consultation fee up front.
Then please bring with you your Medicare Card and a Savings/Cheque Account Debit Card so we can process your reimbursement.
This cannot be a credit card.
You will receive your reimbursement directly to your Savings/Cheque Account.
